Pay attention to the date and time sections. This is vitally important as it gives the staff a definite order of when things were placed in the box.
And please make sure your name and phone number are included as well or you will not get put on the registration. You can download and print this form from home to fill out.

-Some water classes allow deep water participants, if you can swim in deep if needed check the box.
